Mission: To provide comprehensive, compassionate care that enables chronically ill aging adults to remain in their homes and thrive within their communities.

PACE (Program of All-Inclusive Care for the Elderly) Southeast Michigan is a unique health plan and comprehensive care provider, committed to keeping chronically ill aging adults in their home by caring for their medical, psychosocial, and spiritual needs. We are dedicated to addressing the medical, psychosocial, and spiritual needs of each individual, ensuring they receive the extraordinary care they deserve. By fostering a collaborative team environment and committing to continuous growth and improvement, we strive to empower over 3,000 aging adults with chronic conditions to maintain their independence and quality of life.

What We Do

PACE Southeast Michigan helps older adults stay in the homes and communities they love by caring for their medical, physical, social, and spiritual needs. PACE Southeast Michigan provides care and services in the home, community, and our day health centers throughout Wayne, Oakland, and Macomb counties. Participants receive most of their care from our expert staff at our centers. We provide transportation to and from our centers and all medically-necessary appointments.
