Who We Are
We are founded on the belief that our approach to hospice has an enormous impact on those we serve. There is no greater honor than assisting our patients and their families on this end of life journey. Our collaborative approach to hospice services enables our patients and their families to focus on the most important thing: quality of life. The Careline Hospice team works together with our patients along with their support network and care providers to provide a customized plan to meet their specific needs. Our Careline mission is: To Serve Together, Provide Value and Deliver Exceptional Care. We provide quality care anywhere our patients reside. The hospice team, led by a dedicated RN case manager and overseen by a physician, helps our patients and their family make informed decisions about care choices and options. We take the time to understand their goals and strive to reach them.
